We will be documenting the storytelling workshops by recording the zoom session. This is to supply evidence that the work took place for our funders and to share our work to our online community with your consent.
We will not share any photographs or video footage without your consent. Therefore we have provided the form below to be completed as we require your permission to use any footage that you may appear in as a participant of this workshop.
Your names will not be shared, only your likeness as recorded in the zoom session.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on film and through photography during this workshop without any other consideration.
